Blackford Talent is engaged on a Geriatrician search with a well-regarded community health system in Murrieta, CA.

The role

Primary care and geriatrics for seniors who would otherwise be in a nursing home, kept at home instead.

You'd manage the full picture. Multimorbidity, dementia, functional decline, polypharmacy, advanced illness. Goals-of-care conversations, capacity assessments, POLST, and palliative principles worked into routine care.

Care runs mostly through the PACE center, with input on participants in the home, hospital, or SNF when it's clinically warranted.

What makes PACE different

You sit on an interdisciplinary team with nursing, rehab, behavioral health, social work, dietary, home care, pharmacy, and transportation. Same participants, same table, every week.

The program is capitated, which means nobody is counting your RVUs. If a participant needs an hour, they get an hour. Panel sizes in PACE run a fraction of standard primary care. You'll actually know these people.

The organization

A non-profit community health organization in Southern California, operating for more than 50 years. Federally qualified health center running a PACE program alongside its clinics.

Why a physician takes this job

  • No RVU treadmill. Capitated model, small panel, time with participants.
  • A real team. Rehab, behavioral health, social work, and pharmacy in the building and in the room. Not referrals into a void.
  • Predictable hours. Monday to Friday, 8 to 5. No weekends or holidays.
  • Teaching. APP supervision, residents, and rotating students.

What you need

  • MD or DO
  • Active California medical license
  • Completed residency in Family Medicine, Internal Medicine, Geriatrics, or a related specialty
  • Board certified or board eligible
  • Current California DEA registration, maintained as a condition of employment
  • Current BLS through an AHA-approved course (in-person skills check required; online-only doesn't count). CPR and First Aid are also required for all PACE staff per CMS and DHCS. First Aid training is provided at onboarding and renewed annually.
  • Real experience with frail elders, medically complex patients, or chronic illness populations

Preferred: PACE, geriatrics, managed care, home-based primary care, long-term care, or SNF background. Fellowship training in Geriatrics or Palliative Care gets special consideration.
